Original, signed United States of America passport with at least 6 months of remaining validity.

In-Person Submission. Applicants applying for a visa to Angola must submit their applications and documents personally to the Embassy or Consulate. VisaHQ can then collect the passport and ship it back to you after the visa is issued.
Itinerary. Copy of round trip tickets or detailed itinerary for the duration of the trip (Applicant's name must be on itinerary).
Yellow Fever Vaccination.
A copy of the International Certificate of Vaccination (ICV) for yellow fever. To learn more about the yellow fever vaccination or to locate a vaccination clinic, please visit the CDC's yellow fever resource page
Bank Statements. Copy of the applicant's most recent monthly bank statement showing proof of sufficient funds (at least $200 per day). The statement must clearly show the applicant's name as the account holder, the balances of the accounts, and the date of the statement.
Hotel Reservations. Copy of confirmed hotel reservations (Applicant's name must be on Hotel Reservation).
Letter of Request. A letter with a request to issue a visa addressed to the Consular Section of the Embassy of Angola. Please, indicate your name, the purpose of your trip, your contact in Angola, the dates of your planned trip, and places to be visited.
